Before Your First Consultation

1. Obtain a REFERRAL from your General Practitioner

A current referral is necessary in order to claim a Medicare rebate. Referrals from General Practitioners (GPs) are valid for 12 months, although they can also be made indefinite. Referrals from Specialists are valid for 3 months.   Please note that you also need a current Medicare card number to claim also.

Please be aware that you are responsible to check when your referral expires during treatment, as any appointments without a current referral on file will be ineligible for a Medicare rebate, and any testing done onsite will be privately billed rather than bulk billed.

2. Send us your referral

Please SEND your referral to us PRIOR to contacting us to make an appointment.   Your referral needs to state the condition/s of which you are wanting to be assessed and treated.  This is to allow our reception staff to determine what type and length of appointment you require.

There are three ways you can send us your referral.
1. Fax:           30538124
2. Email
:         This e-mail address is being protected from spambots. You need JavaScript enabled to view it
3. Post:          HEALTH & DIABETES CENTRE

Suite 44, Level 4
                        101 Wickham Tce, Springhill Q 4000

3. Forms to fill in prior to initial attendance

After your referral has been received & you have been scheduled to attend our Clinic, we will forward
your 'Clinic Pack' to you via email or post.  This will include an introductory letter with your appointment details, information on appointment fees & protocols, plus some forms that you are required to fill in and return to the Clinic prior to your first attendance.   If we do not receive the forms you will need to use your first Clinic attendance  to do so, as the information gathered in these forms is necessary for your initital assessments.
